I’m a female & ended up cutting off my oldest childhood “best friend” for exactly this reason. At least your hoe friend is a naive immature 21 year old. Mine was still on this behavior in her late 30s. She was never able to admit her insecurities/problems and jump out of her cycle of self destructive addictive behavior. I haven’t talked to her in 2 years and she’s still on the same BS “dates.”

I don’t think expecting that common areas of the home be kept clear of clutter. Setting up a man cave/craft room downstairs seems smart. Then he never has to clear it and you don’t have to look at it.
Personally, if he won’t pick up just out of respect for your wishes then….I guess you can do what I did with the kids. When they went to bed and I was tidying up, anything that wasn’t put away where it belonged or left out went in a basket and they could earn the things back by doing an extra chore.

Let's don't forget that girl was also a hyper-sexual child and almost certainly had been SA'd herself. While she DID victimize your BF and nothing makes that okay, she is almost certainly someone else's victim.
Finding a therapist that clicks sometimes takes some trial and error, encourage him to try again.
