I doubt he meant to send this to your sister, unless he really is incredibly stupid. He had to have known your sister would tell you. My guess is that he meant to send it to someone else entirely. He very well may be physically cheating, or at least sexting with someone (which obviously is cheating as well). If it was not for the fact that you said that he never sends pics like this to you, I'd say maybe he was drunk and missed you and…. but the fact that this is out of character makes me feel like you were not the intended recipient. If I were you I would be looking for other evidence of cheating.

There was a book rec on the women's subreddit called The Authority Gap. It's a good book an very thoroughly breaks down how women are just taken less seriously than men, even women in higher positions, even women who are older or more accomplished or women who are correct every time they say, “that doesn't sound quite right”. Some men are not bad at taking women seriously and treating their thoughts and knowledge with respect. Your boyfriend is not one of them.
