A lot of people have depression and anxiety, but yet still drive and go to work. It sounds like she's lazy and is taking advantage of you. If she won't get help for her issues, then it's time for you to go. Either that or just stop paying for things for her.
I don't think it is a question of “looking best”, but the fact that she has no economic security and is compromising what little she says. I would be remiss if I didn't say something.
