Birthdays are for young people. Many adults don't do anything with them. This is part of the transition to adulthood.
To be fair, it is possible he sent the dickpic to the sister by accident and it was meant for another woman named like the sister. And yes, he is a pig and he is definitely cheating. Don't change your relationship with your sister, the fact she is more beautiful (or not) than you has nothing to do with this. And talk to a lawyer to divorce your husband. Perhaps he doesn't want a divorce because he is afraid he will lose some money but for sure he no longer wants a relationship with you
