On my humble opinion, people are not entitled to forgiveness in anything. If someone hurts you and try to make amends, is up to you to forgive and forget or not. With that in mind, no, cheating should not be forgiven; it can be forgiven if the person that was cheated feel like it.
I personally don't forgive cheaters because I have this amazing premise: if someone disrespect you once and you accept it, they will do again eventually. And cheating is the primary form of disrespect and betrayal. If my partner wants to fuck around, he can break up, he can discuss an open relationship, he can tell me and see if we can work on it. He can't do that behind my back, deceive me and then say he is sorry for hurting me deliberately.
So yeah for me is at least an yellow flag. I would not give ultimatums here because if he is cheating me lashing out would only make him a better liar, but I would ask more about it. Does cheaters should always be forgiven? Would he forgive you if you cheated on him? Did he ever cheated a girl? What he considered cheating? And work with that information.
