I think that this is a two way problem. In the first example, I don't think that you should expect her to take on blame and be sorry for something that she clearly didn't mean. You need to understand your own insecurities in order to understand yourself better and move past them. You can't expect others to be constantly mindful of them and apologize when they have a normal conversation with you. Saying that she didn't mean it that way should be enough, especially if you know that's not what she meant already.
